The Sanguinesti staff is a one-handed powered staff requiring 82 Magic to wield, dropped from the Theatre of Blood. It fires a built-in blood spell at 4 ticks (2.4s) with a base max hit of 26 at 82 Magic, and carries a unique healing passive that restores HP on a portion of your hits. That self-sustain makes it a staple at long PvM encounters like Zulrah, the Kraken and the Chambers of Xeric. It is untradeable once charged; the ~20.7M price shown is its uncharged tradeable form.

How to get the Sanguinesti staff

The Sanguinesti staff is a rare unique reward from the Theatre of Blood, the raid in Ver Sinhaza. It cannot be bought directly in its charged form — you either earn it from the raid or buy the uncharged twin off the Grand Exchange.

  • Raid drop: A completed normal-mode raid has roughly an 11% chance of rolling a unique reward, and the staff is one of the unique items on that table (alongside the rapier, scythe, Justiciar armour and the Avernic defender hilt). Hard mode raises the unique chance to about 13%.
  • Grand Exchange: Players who don't raid usually buy the uncharged Sanguinesti staff, which is tradeable, then charge it themselves with blood runes.

Charges and the healing passive

The Sanguinesti staff is a charged item. To cast its built-in spell it must be charged with blood runes — each cast uses 3 blood runes, and the staff holds up to 20,000 charges (60,000 blood runes) when fully topped up. You can charge or uncharge it any time, and uncharging returns all of the blood runes, so the staff loses no value sitting in the bank.

This is why two versions exist. The charged staff (the one you wield) is untradeable; only the uncharged form can be traded. The live price and chart on this page track that uncharged tradeable item — around 20.7M — so the price you see is the staff before any blood runes are loaded into it.

The staff's signature feature is its healing passive: a successful hit has a 1/6 (~16.6%) chance to heal you for half the damage that hit dealt, which works out to roughly 8.3% of your total damage returned as hitpoints over time. That free sustain is what sets it apart from the tridents and makes it a go-to mage weapon at drawn-out fights.

Is the Sanguinesti staff worth it?

For mid-to-late-game magic PvM, the Sanguinesti staff is one of the best value weapons in the game. The built-in blood spell hits one point higher than the trident of the swamp (base max 34 versus 33 on the standard spell), and the healing passive keeps you topped up so you burn fewer brews and sharks at sustained encounters like Zulrah, the Kraken, Dagannoth Rex and the Chambers of Xeric.

It is outclassed at the very top end by Tumeken's shadow, which has a higher base max hit and triples magic bonuses — see Tumeken's shadow if you're chasing the absolute best. But the shadow costs many times more, so for most players the Sanguinesti staff is the sweet spot: strong, self-healing, and far cheaper. Because it's untradeable when charged, you buy the uncharged form on the GE price tracker and charge it yourself.
