The occult necklace is a drop from smoke devils, which live in the Smoke Devil Dungeon and need 93 Slayer, an active Slayer task, and a facemask or slayer helmet to harm. Regular smoke devils drop it at 1/512.
Its boss variant, the thermonuclear smoke devil, drops it at a better 1/350, and the superior nuclear smoke devil drops it at 1/512 across three rolls.
