Its real draw is its special attack, Shove, which knocks a target back and stuns them, making it a popular PvP utility weapon rather than a damage dealer. It's a rare drop from the rare and gem drop tables and from a few monsters, and is one of the cheapest dragon weapons going.

How to get the dragon spear
The dragon spear is not made with Smithing — like most dragon equipment it can only be obtained as a drop or bought on the Grand Exchange.
- Rare drop table — accessible from most Slayer monsters; no quest needed.
- Gem drop table — but only after completing Legends' Quest, otherwise a talisman drops in its place.
- Goraks — drop it at 1/325 when a ring of wealth is equipped.
- Brutal black dragons — drop it directly at 1/512 (plus their own rare-drop-table access).
Ironmen also chase it for clue scrolls: two elite Treasure Trail emote steps require a dragon spear. Note that a poisoned dragon spear will not satisfy those clue steps — only the plain version works.
Special attack: Shove
The dragon spear's special attack, Shove, costs 25% special energy and is shared with the Zamorakian spear and hasta. It pushes the target back and stuns them for 5 ticks (3.0 seconds), during which they cannot act. The special itself swings on a 5-tick timer, slightly slower than the spear's normal 4-tick (2.4-second) attack.
It deals no real damage, so it is a control tool for PvP: while a target is stunned all incoming hits are stalled, then land at once when the stun ends. The effect doesn't stack on an already-stunned player, and a target gets a brief immunity window after each stun. Shove cannot be used on large monsters — anything bigger than one game tile (giants, dragons) is too big to knock back, and the special simply fails with no energy spent.

Update history
The spear's attack speed was increased from 5 (every 3.0 seconds) to 4 (every 2.4 seconds) in the Equipment Rebalance.
You can no longer use the Dragon spear, Zamorakian spear or hasta special attack on a target who is already stunned, and players get a 1-tick immunity period after a stun wears off.
The poisoned variants were renamed: "Dragon spear(+)" to "Dragon spear(p+)" and "Dragon spear(s)" to "Dragon spear(p++)".
