The Basilisk jaw is a rare drop from Basilisk Knights that upgrades a Helm of Neitiznot into the Neitiznot faceguard. It is not worn or used in combat itself. It only matters as the single upgrade part, and it needs The Fremennik Exiles to be finished before you can do anything with it.

How to get a Basilisk jaw
The jaw is a drop only, and a very rare one.
Ways to obtain it
Basilisk Knights — 1/5,000, or 1/1,000 while you are on a basilisk Slayer task. They are found in Jormungand's Prison and need 60 Slayer and The Fremennik Exiles.
Basilisk Sentinels — the superior version of the Basilisk Knight, which can appear once you unlock Bigger and Badder. Drop rate 1/1,000.
Grand Exchange — the jaw is tradeable, so you can buy it outright.
You must bring a mirror shield or V's shield to every kill, or the Knights drain your stats.
What the Basilisk jaw makes
The jaw has one use. After The Fremennik Exiles, put a Helm of Neitiznot and the jaw in your inventory and ask Olaf the Bard for stories about basilisk jaws. You get a Neitiznot faceguard.
The faceguard needs 70 Defence and the same quest to wear. It is untradeable, but you can turn it back into the helm and the jaw whenever you like.
Is the Basilisk jaw worth it?
Compare the two helms first. The Helm of Neitiznot has defences of +31 stab, +29 slash, +34 crush, +3 magic and +30 ranged, with +3 strength and +3 Prayer. The faceguard raises those to +36, +34, +38, +3 and +34, doubles the strength bonus to +6, and keeps the +3 Prayer.
That is a modest gain for the price of the jaw, so the faceguard is a luxury upgrade, not a needed one. It suits a player with a lot of spare coins who already wears the helm. For the best head slot overall, see the melee gear page and the best in slot gear list.
Basilisk jaw FAQ
What is the Basilisk jaw used for?
It upgrades a Helm of Neitiznot into a Neitiznot faceguard. You do this by asking Olaf the Bard for stories about basilisk jaws while the jaw is in your inventory.
What do you need to make the Neitiznot faceguard?
You need the jaw, a Helm of Neitiznot, and The Fremennik Exiles completed. Wearing the faceguard needs 70 Defence and the same quest.
Where does the Basilisk jaw drop?
From Basilisk Knights, and from their superior version, the Basilisk Sentinel, if you have unlocked Bigger and Badder. The Knights live in Jormungand's Prison under the Island of Stone. You need 60 Slayer and The Fremennik Exiles to fight them.
What is the Basilisk jaw drop rate?
The Basilisk Knight drop is 1/5,000, which improves to 1/1,000 when you are on a basilisk Slayer task. The Basilisk Sentinel drops it at 1/1,000.
Can you trade the Basilisk jaw?
Yes. It is tradeable, so you can buy one on the Grand Exchange instead of farming it. The faceguard you make from it is untradeable.
Can you undo the Neitiznot faceguard?
Yes. The faceguard can be reverted at any time. It is also converted back if you lose it in PvP.
What do you need to fight Basilisk Knights?
You need a mirror shield or V's shield. Without one you take heavy damage and your stats are drained.
Is the Neitiznot faceguard worth the cost?
It adds defence and doubles the strength bonus of the Helm of Neitiznot, from +3 to +6, but the jaw is a large investment. It is a late upgrade for a player who already uses the Helm of Neitiznot. See the worth section for the numbers.
