It attacks every 4 ticks (2.4s) and brings +121 crush, +115 stab and +81 strength, but its defining trait is that it hits twice per attack in two halved hitsplats. It is obtained as a possible reward from the Lunar Chest in Neypotzli at a 1/224 rate. The market price for one sits around 5.5M.

How to get the dual macuahuitl

The dual macuahuitl is not a drop from a regular monster — it is a possible reward from the Lunar Chest in Neypotzli, the reward chest of the Moons of Peril trio bosses introduced in Varlamore: Part One.

  • Its unique drop rate is 1/224 per chest opening. That base rate does not account for the chest's duplicate-protection system, which makes the practical experience smoother once you have collected some of the moon-set uniques.
  • That 1/224 chance is rolled when you defeat the Blood Moon specifically — each Moon is rolled independently — so the macuahuitl shares its drop slot only with the other Blood Moon uniques (the blood moon armour set), not the eclipse or blue moon rewards.
  • Because it is tradeable, you do not have to grind the chest yourself — you can buy one on the Grand Exchange for roughly 5.5M, with a buy limit of 15 every 4 hours.

How the two-hit mechanic works

The dual macuahuitl hits twice per attack, with the two hitsplats spaced one game tick apart when attacking monsters. Your maximum hit is split across the two hits — both halved — and if your max hit is odd, the first hit rounds down and the second rounds up (a max of 25 becomes 12 and 13).

The catch is in the accuracy. Unlike most two-hit weapons where each hit rolls independently, the dual macuahuitl checks its two hits sequentially: the second accuracy roll only happens if the first one succeeds. If the first hit misses, both hits deal no damage. This is intentional — it pushes the weapon into a niche against targets with lower Defence levels, where the first check lands often, and weakens it against high-Defence targets.

To compensate, the weapon carries unusually high attack bonuses for its level requirement and speed. The wiki notes its crush bonus substantially exceeds that of the abyssal bludgeon and rivals the combined crush bonus of an inquisitor's mace and avernic defender.

Special attack & set effect

The dual macuahuitl has a special attack, Blood Infusion, costing 25% special attack energy. It can only be used while wearing the full blood moon armour set.

  • Blood Infusion removes the sequential accuracy check (so both hits roll normally) and raises your minimum and maximum hit by 25% for that attack.
  • It costs you HP: you take damage equal to 25% of your current Hitpoints, rounded down (no self-damage below 4 HP). If the special misses, you still take the damage.
  • While wearing the full blood moon set, the special also guarantees the Bloodrager set effect triggers when the attack hits.

The blood moon armour set also grants the Bloodrager set effect: a 33% chance on a successful hit to attack one tick earlier than usual (attacking again after 3 ticks). Because the weapon hits twice, each hit can roll for it independently, giving roughly a 55.5% chance to trigger when both hits land. Note: an amulet of blood fury drains its charges quickly here, since you land two hits per attack and attack faster under the set effect.
